I simply use a slightly larger hook and make a copy of the first part of the head up to where the ears would go approx. then fit it on the doll like a hat and stitch it all around the edge so it fits like a natural hairline.  Then I hook the hair through the cap stitches, rather than directly onto the head stitches.

I make a hair cap.  Crochet the same pattern as for the head until it´s the size of a natural hairline in the hair yarn, but use a bigger hook. Sew the cap onto the head and then use the stitches of the the cap, rather than the head for adding the hair.  Hope this makes sense.  I did this for these fairies -
