This chart is spelled wrong and doesn't have the designer name. It correct spelling is The Plymouth Sampler and the designer is Brenda Keyes. The information also says it's worked in half stitches but it can be worked in half stitches or whole cross stitches.  It also gets blurry when enlarged but I was able to read it enough. :)
