I usually try to stitch over 2 threads to avoid the problem. If stitching over 2 threads, when you start, make sure you start "over the post", there's a thread on here somewhere with diagrams, when you pull the thread up in the lower left corner you should go over a vertical thread then down in the upper right. It sounds like your tension may be too tight.  Have you tried using a hoop?

Don't use the "sewing" method, instead push the needle down, pull the thread all the way through, then come up from the back separately. I usually try to stitch over 2 threads to avoid the problem. If stitching over 2 threads, when you start, make sure you start "over the post", there's a thread on here somewhere with diagrams, when you pull the thread up in the lower left corner you should go over a vertical thread then down in the upper right.

Another tip is not to tighten your thread when stitching. Or use 3 threads instead of 2.
