How much fabric do you normally leave around the design? Would 3" be normal or 4.5" be considered normal?

I do 3 inches and have never had a problem framing my work

It depends if you are to finish the stitched piece into an object or for framing. If its for framing, you need to take into consideration the border allowance for a matting board if you use one other than the frame.
